What is CoolSculpting?
Two Harvard scientists were the inventors of CoolSculpting. Their revolutionary idea came as a byproduct of research they were conducting on the development of dimples in children who ate lots of popsicles. They theorized that the cold of the popsicles was somehow limiting or damaging the fat in the kids’ cheeks, creating the dimples. If cold affected fat in the cheeks, maybe it would do so elsewhere, such as the fat pockets that form on the abdomen. CoolSculpting was the result.
In their research, they discovered that fat cells freeze at a higher temperature than other tissue cells. So, CoolSculpting delivers temperatures low enough to freeze the fat cells in the treatment area, leaving the adjacent non-fat tissue cells unharmed. This freezing crystallizes and kills 20 to 25 percent of the fat cells in the treated area. The body then flushes the dead cells from the body over the following weeks.
CoolSculpting vs. liposuction
Both of these procedures aim to remove fat in the target areas, but they go about it in completely different fashions. Liposuction is surgery. It involves the creation of small incisions through which a cannula (a small tube with suction) is inserted. The cannula is moved back and forth to break loose and then suction out fat cells. Because of the movement, there is some skin trauma and subsequent bruising afterwards. There is also some recovery required, especially if you have larger areas, such as the outer thighs, treated with liposuction.
CoolSculpting is completely non-invasive. By using controlled freezing of fat cells, CoolSculpting eliminates 20-25 percent of the fat in the treatment area. There are no incisions, no skin trauma. As soon as your CoolSculpting session is over, you can go right back to your normal activities. There is no recovery.
In the degree of fat removed, liposuction can remove more fat more dramatically (although it is not a weight-loss treatment) than CoolSculpting, but at times removing too much fat can create loose skin on the surface.

How does a CoolSculpting session go?
When you come to our office for a CoolSculpting session, there’s no need for any local anesthetic or pain relief. You simply sit in our angled treatment chair and we position the CoolSculpting panels over the treatment area. The panels produce light suction that draws the skin, underlying tissue, and fat up between the two cooling panels. We then set the temperature to the freezing point of the fat cells and deliver that cooling down into the drawn up skin and tissue. The fat cells freeze and the body begins the process of eliminating them. As the session progresses, we repeat this process several times across the treatment area. A total session takes from one to three hours, depending on the size of the treatment areas.
Over time, once 20-25 percent of the fat in the area is gone, the remaining fat cells condense down into a thinner layer and your contour slims.
CoolSculpting is completely non-invasive. There are no incisions of any sort. After treatment, patients usually have some degree of redness, slight swelling, bruising, and skin sensitivity on the areas where the CoolSculpting applicators drew the skin onto the cooling panels. These after-effects are due to the outer skin reacting to the intense cold delivered in a CoolSculpting session. They usually pass within a couple days to a week at most. We’ve found that these feelings and sensations don’t keep our patients from returning to work or their normal activities immediately.

At Dermatology Institute & Skin Care Center, we generally want our CoolSculpting patients to be within 10 percent of their ideal body weight to have this procedure. Other than that, there really aren’t many restrictions on who can have CoolSculpting done. The only people who absolutely cannot have CoolSculpting are those suffering from cryoglobulinemia, cold agglutinin disease, or paroxysmal cold hemoglobinuria.

Is fat reduction painful?
You won’t feel pain during your session. You’ll feel a pulling sensation as the suction draws your tissues up to the cooling panels. Once the cooling is delivered, at first you’ll feel an intense cooling sensation for a few minutes. Most people don’t describe this is painful, just very cold. Then the area becomes numb and the sensation passes. From there, you’ll feel little. You can read a book, answer emails, watch videos on your laptop, or take a nap during the procedure.
Is there downtime with CoolSculpting?

You won’t see immediate results with CoolSculpting. This is because, while the fat cells have been frozen and are dying, it takes time for the body to remove them. Some patients say they begin to see changes in around three weeks, but the most dramatic results will show themselves after two months or so. And these results keep improving for up to six months.
Are CoolSculpting results permanent?
The fat cells that are frozen and die are gone for good. That fat reduction is permanent. If you don’t gain a large amount of weight, your results should last forever. If you do put on weight, it tends to build elsewhere before returning to the sites where you’ve had CoolSculpting.
What happens if I gain weight after my CoolSculpting treatment?
The fat that is destroyed and flushed from the body is permanently gone. That means the treatment areas will have roughly 20 percent less fat…for good. If you gain a significant amount of weight down the road fat can return to these areas, but it usually accumulates across the body. Most patients, once they see the changes CoolSculpting has made to their body contour, begin to take a more serious look at their diet and lifestyle and make changes to keep the weight off.
CoolSculpting vs. Kybella for double chins
Both CoolSculpting Mini, which uses an applicator to address smaller areas such as under the chin, and Kybella, which is an injectable that kills fat cells under the chin, attack and kill fat cells. In that way, they are the same. Both methods will probably also require more than one treatment session to adequately address the patient’s double chin.
The difference is the method. If you don’t care for injections, you would prefer CoolSculpting. CoolSculpting may also be more effective if the patient has a large amount of fat under his or her chin.
Kybella is given in up to six total treatments; each treatment session involves multiple injections with a very tiny needle. Kybella, which is basically deoxycholic acid, then breaks down fat cells and dramatically slims the profile. Kybella is superior for very targeted pockets of fat. Kybella seems to create a more defined profile under the chin than CoolSculpting.
